PLEASE READ INSTRUCTIONS AND THEN SCROLL DOWN FOR THE SUBMISSION FORM.

Our goal in collecting this data is to understand the frequency of various infectious diseases, environmental toxins, and parasites in Central Oregon. This will help us all provide our customers with the best possible medical advice specific to our location. It will also help track outbreaks, especially if they are not reportable to the state. Information specific to clinic/doctor name will not be published.

When entering the “Diagnosed disease or toxin” in the form below, please do not capitalize unless you are recording a disease commonly referred to with an acronym like CIRDC. This is to make it easier to sort and organize data.

Please see below for more guidance on how to input your cases.

  • When recording an infectious disease, please record the organism rather than the disease (aka. histoplasma vs. histoplasmosis). Examples of infectious diseases: leptospirosis, histoplasmosis, leishmaniasis, influenza, etc.

    • For CIRDC (kennel cough), if you know the causative organism, please record as CIRDC (organism). Example: CIRDC (bordetella) or CIRDC (influenza). Similarly, for feline URI, please record as URI (organism). Example: URI (mycoplasma) or URI (calicivirus).

  • When recording parasites, please report the parasite common name. Examples of parasites: heartworm, hookworm, whipworm, etc.

  • When recording tick-bourne diseases, please report the organism rather than the disease (aka. ehrlichia vs. ehrlichiosis). Examples of tick-bourne diseases: ehrlichia, lyme, anaplasma, etc.

  • When recording an environmental toxin, please report the toxin itself. We do not need to collect information on common toxins like marijuana. Examples of toxins: blue-green algae, mushroom, etc.
